ATTENDANT CARE
·  Must meet financial and medical eligibility
·  Age criteria:  18-59
·  Must be able to direct his/her own services
·  Require personal care assistance (ADLs)
·  Health Maintenance Activities
·  Recruit, hire, train & supervise PC Attendant

CSPPPD - INDEPENDENCE WAIVER
·  Provides services to eligible persons with physical disabilities in order to prevent institutionalization and allows them to remain as independent as possible
·  Age 18 and older
·  Severe physical disability which is likely to continue indefinitely
·  Results in substantial functional limitations in three or more major life activities
·  Eligible for nursing facility services
·  Primary diagnosis can not be mental health or mental retardation

CSPPPD - COMMCARE WAIVER
·  Must have a diagnosis of  Traumatic Brain Injury (TBI) as the primary diagnosis on the MA-51.
·  Services may only be provided to individual living in a house, apartment or licensed facility of six(6) or fewer beds
·  Consumer must require a special Rehabilitation Facility Level of care (SRF)
·  Must be 21 years of age or older
·  Not dependent on mechanical ventilator supports
·  Results in substantial functional limitation in three or more of major life activities
